Consultation and Appointment Reservation Information
For first-time patients
If you are a first-time visitor, please be sure to bring your health insurance card and any other medical welfare beneficiary cards.
If you can bring in your medications notebook and prior medical test records, we will add them to our files for a more complete reference of your treatment.
If you have a letter of introduction from another hospital, please be sure to bring it in and give it to the Doctor.
A clinic Patient Card (medical examination ticket) will be issued to you. Please bring it with you whenever you visit our clinic going forward.
First-time Patient Reservations:
You may make an online reservation to help us make patient flow more efficient. Walk-ins are welcome as well (no appointment necessary).
For Returning Patients
If one month has passed since your last visit, or if your health insurance card or information has changed, please report it to the front desk. We will update your health insurance card accordingly.
Please make a reservation and fill out the questionnaire online to ensure your visit goes smoothly and reduce your wait time.
For Day Surgery Patients
If you wish to schedule a day surgery, please make an appointment for a medical examination with us first.
The Doctor will examine your condition and set the examination method, treatment, surgery and post-surgery checkup schedule after a thorough consultation.
